• Office hours are Monday - Friday 9 AM - 4:30 PM by appointment only.

• All referrals are the responsibility of the patient and must be obtained prior to the scheduled appointment.

• Payment and co-payments are expected at the time of the visit and can not be billed.

• 24 hour notice is required to cancel all appointments and procedures.

• Prescription renewals should be handled during regular office hours listed above.

• Please bring all insurance cards and paper referrals, if applicable, along with any relevant medical records (prior procedure reports, x-ray reports and biopsy results).
